Maxillofacial surgery Techniques and Specialized Procedures

Maxillofacial surgery (Orthognathic) is a surgical specialty that addresses pathologies, injuries, and deformities of the craniofacial region.

Core techniques include orthognathic surgery (correcting overbites, underbites, and facial asymmetry), trauma treatment, dental implants, and microsurgical reconstruction.

Orthognathic Surgery

Intensive interventions to correct severe malocclusion and facial bone structure abnormalities.

Le Fort (Upper Jaw Shifting): Moving the upper jaw in three dimensions to correct overbites or gum recession.

BSSO (Body Slicing and Splitting of the Mandible): Splitting the mandible vertically to reposition (correct underbites) or slide the chin forward.

Genioplasty (Genioplasty): Removing a portion of the chin bone and fixing it in a new position to lengthen, shorten, or create a balanced projection.

Surgery for Trauma and Pathology

Procedures to address the consequences of accidents or tumors and cysts in the maxillofacial region.

Maxillofacial Bone Fixation: Correction and fixation of fractured bone segments using titanium plates and screws (treatment of mandibular fractures, zygomatic fractures, and orbital fractures).

Tumor and Cyst Removal Surgery: Removal of jaw bone cysts, benign or cancerous tumors in the oral cavity.

Abscess Drainage: Incision and drainage of pus from severe infections (such as floor of the mouth abscesses, submandibular abscesses).

Dental Implants and Similar Surgery

Procedures for restoring chewing function and periodontal tissues.

Implant Placement: Placement of titanium posts into the jawbone to replace lost tooth roots.

  • Bone Grafting and Sinus Lift: Autogenous or artificial bone grafting, maxillary sinus lift to create sufficient bone volume before implant placement.

Microsurgery for Reconstruction and Congenital Deformities

Complex interventions to reconstruct large facial defects

Free flap: Transferring a flap of skin, muscle, or bone (such as a fibula flap) from another part of the body to the maxillofacial region, connecting blood vessels under a microscope to reconstruct a defect caused by cancer or severe trauma.

Cleft lip and palate surgery: Procedures involving dissection and suturing to close congenital clefts in children.

Post-operative Care and Recovery after Jaw Surgery

Proper post-operative care after maxillofacial surgery helps minimize swelling and pain, prevent infection, and speed up recovery. The focus includes: applying cold/warm compresses to reduce swelling, maintaining oral hygiene with antiseptic mouthwash, and keeping a soft, liquid diet during the first few weeks

Managing Swelling and Pain

  • Ice compresses: For the first 48 hours, apply an ice pack (wrapped in a towel) to the swollen area of the face every 15-20 minutes to constrict blood vessels, reduce bruising and swelling.
  • Warm compresses: From day 3 onwards, switch to warm compresses to help dissolve blood clots.
  • Medication: Take pain medication and antibiotics as prescribed by your doctor. Do not self-medicate.

Oral Hygiene

  • Mouthwash: For the first 7 days, avoid brushing directly over the surgical wound. Use a specialized mouthwash (such as Chlorhexidine) or saline solution to gently rinse after meals to disinfect.
  • Hygiene: You can use cotton swabs soaked in saline solution to gently clean the spaces between your teeth and the outer areas of your mouth and lips.

Diet

  • Soft, liquid foods: In the first week, prioritize milk, blended soups, thin porridge, and smoothies.
  • Avoid jaw movement: Absolutely avoid hard, chewy, sticky foods or foods that require strong chewing force for at least the first month to allow the jawbone to fully recover.
  • Food temperature: Do not eat foods that are too hot or too cold, as this can cause irritation or bleeding.

Activities and Rest

  • Sleeping: Elevate your head with a pillow (about 30 degrees) when sleeping in the first few days to prevent blood from pooling in your face and reduce swelling.
  • Rest: Take time to rest and recuperate, avoid talking too much, laughing loudly, or doing strenuous work.
  • Abstinence: Stay away from alcohol, beer, cigarettes, and stimulants for at least 5 weeks to avoid hindering the healing process.
